The Bipartisan Policy Center has announced new report, Challenges and Opportunities in Care for High-Need, High-Cost Medicare Patients. This report explains the focus on high-need, high-cost Medicare beneficiaries and individuals with multiple chronic conditions. It states "traditional Medicare fee-for-service reimbursement does not promote evidence-based, patient-centered care for patients with multiple chronic conditions such as diabetes and heart disease."  The report further recognizes federal laws and regulations that hinder the integration of care. The Bipartisan Policy Center seeks input from health care providers, policymakers, and other experts on solutions to address these problems.
