In honor of our friend and CityMatCH Board member, Carole Douglas, who lost her battle with cancer on March 18, 2004, a memorial scholarship was established. The scholarship was created in 2004 to financially assist CityMatCH member representatives and emerging leaders so they can attend the Annual CityMatCH Urban Maternal and Child Health (MCH) Leadership Conference.

• All CityMatCH Health Department member representatives may apply for the scholarship.
• Special consideration will be given to those who have not previously attended a CityMatCH conference, or have not attended in the past two years due to financial reasons.
• One stipend of up to $1,000 will be awarded to reimburse for conference-related expenses.

• Interested member representatives must complete the attached application, and provide:
  1. Contact and demographic information,
  2. An essay of no more than 500 words,
  3. Certification and release agreement.
• Completed applications can be e-mailed to: citymch@unmc.edu (subject line: Carole Douglas Memorial Scholarship Application), or mailed to CityMatCH at UNMC, 982170 Nebraska Medical Center, Omaha, NE 68198-2170.
• Applications will be reviewed by a selection committee.
• Upon selection, the recipient must confirm plans to attend the conference.
• The deadline to submit applications is 5:00pm CT Monday, July 21, 2008.
• The scholarship awardees will be acknowledged during the Awards Luncheon on Monday, September 22, 2008 at 12:30 pm.
